Lakeside Psychological Evaluations and Family Consulting, LLC is dedicated to maintaining each client's privacy. A copy of the confidential disclosure agreement is provided at the first session following signature by the client
and/or legal guardian(s).
Wisconsin state law and professional ethics requires that confidentiality be maintained except for the following situations:
- Suspected past or present abuse or neglect
- If it is believed that the client is in danger of harming him/herself or has threatened to harm another person
- If a written release of information has been obtained from the legal guardian(s)
